Woodland Wellness Centre (WWC)

Fill out the COMPLETED application form with a Referring Agent (RA) - Addictions worker, Mental Health Therapist - and send by fax or email; this includes the medical form from your doctor or nurse practitioner. Referring Agent must be included in all correspondence, as a Treatment plan will need to be in place for the participant to be accepted into WWC. The RA will need to fill out the intake package with the participant. If you do not have an RA, please contact the Intake Coordinator for support at: intake@woodlandwellness.ca
Once the application has been received by WWC, it will then go to the review panel for review. The review panel includes Elders, Wellness Workers, LPN, Intake Coordinator, Prevention & Recovery Manager and/or WWC Director.
Participants must be detoxed for a minimum of 7 days before coming to WWC, as they will be urine screened for alcohol and drugs at the point of entry. IF a participant tests positive, they will be sent back home. (Please make sure your ride waits outside for you while you wait for the lab results).
Medical, dental, and legal appointments must be taken care of before admission. You and your RA are required to ensure these appointments do not occur during your treatment stay. This helps you focus on Healing Sessions and land-based activities/events daily.
